Asking patients and families what matters most to them is critical to engaging them in care. Asking patients and families about their views of their health needs, understanding the context of their lives, and learning about their values and preferences are important steps in effectively engaging families in their care and providing effective care. Understanding what matters most to patients is crucial to helping them to develop a care plan and follow through with self-care. Army Medicine’s number one priority to our Soldiers and Family members is to provide easy access to the best quality health care possible while maintaining Soldier medical readiness. Our goal is to have the right provider, providing care at the right time, using the right venue that’s best for the Family members by making primary care services easier to obtain. Community Based Primary Care Clinics improve access to primary care by reducing reliance on the emergency room for routine health care needs.

In addition to Primary Care, these services will also be provided:

Immunization Services – Immunization services are preventive services determined by the patient’s care team based on age, gender, or risk factors.

Pharmacy Services – Pharmacy services include medication dispensing for new prescriptions and refills (enrolled beneficiaries only), patient education, individual and group consultations, medication reconciliation, and prescription monitoring.

Laboratory Services – This is a moderate complexity lab. Laboratory services include phlebotomy, specimen processing, and results reporting.

Radiology Services – Because patients must be sent off-site for radiology services, the clinic ensures that delivery of radiology services is seamless. This means that patient instructions are clear, that the receiving radiology center is informed of the patient’s requirements, that the patient knows what to do after the study is complete, and that results are immediately available to the ordering clinician.

Behavioral Health Services – Scope of care includes assessment, diagnosis, and brief counseling for common primary care diagnoses (anxiety, depression, adjustment disorders, stress reactions, etc.) and behavioral modification related to chronic disease and prevention.

Richmond Hill Medical Home FAQ’s

Below is a list of frequently asked questions and answers regarding the new Richmond Hill Medical Home. If you have additional questions please feel free to contact us directly so that we may assist you with your concerns.

1. Q: What services will be provided by RHMH?
A: Primary care, lab, immunizations, behavioral health and pharmacy services.

2. Q: Who can be seen at RHMH?
A: RHMH will enroll Active Duty Family Members. Our initial focus is those ADFM within a 30 minute driving distance to the Richmond Hill Medical Home.

3. Q: How do I enroll in RHMH or change my PCM to RHMH?
A: Active Duty Family Members, please visit the Ft. Stewart’s TRICARE Service Center, Bldg 202, 872 Harmon Avenue, Ft. Stewart, GA 31314 (Monday – Friday 7:30 a.m. – 4:30 p.m.) or the Hunter Army Airfield’s TRICARE Service Center, Bldg 1440, 230 Duncan Drive, HAAF, GA 31409 (Monday – Friday 8:30 a.m. – 4:00 p.m. (closed every 4th Thursday until 1:00 p.m.) and complete application and/or PCM change request.

4. Q: Can I get a same day appointment?
A: Yes. Our goal is to take care of you and your family’s needs. In the event we are unable to meet your request, your Team Nurse may call to speak to you regarding your needs and refer you to your Team provider for care as appropriate.

5. Q: Can I get back to back appointments if I have multiple family members to be seen?
A: Yes, when time allows we will book back to back or a group appointment. Our goal is to take care of you and your family’s needs.

6. Q: How will follow-up appointments be handled?
A: Follow up appointments can be made before leaving. Check with your health care team if you have a referral for specialty care as your team nurse may be able to facilitate an appt prior to your departure.

7. Q: Can I request the gender of my provider?
A: Yes, we will aim to meet the needs of each family member. However, the provider will be chosen to best suit your medical needs.

8. Q: Can I call directly to my health care team?
A: Yes, once Richmond Hill Medical Home is open, you may come to the clinic and pick up a card with all your health care team information.

9. Q: If I need a referral, who will take care of this?
A: Your health care team will take care of your referral during your visit. You may check on the status of your referral after 3 days by contacting our Referral Management Office at 912-435-6015.

10. Q: Can I get my medication refills at RHMH?
A: Yes, as long as you are enrolled in RHMH. Presently, clinics are planned to have a basic Primary Care formulary. The formulary will not include all the prescriptions that may be carried at the parent MTF but will carry prescriptions that will cover the gamut of primary care medicine. The clinic will concentrate on ensuring that the enrolled beneficiaries have the proper medications to meet the primary care mission of these clinics.

11. Q: If I am Active Duty and my family is assigned to RHMH, can my spouse pick up my medication along with theirs?
A: No, the pharmacy within RHMH is currently only equipped to fill prescriptions for Active Duty family members. Thus you will still need to pick up your medications and refills at your current designated location(s).

12. Q: Can I pick up my medication and/or refills at RHMH if I am a retiree?
A: No, the pharmacy within RHMH is currently only equipped to fill prescriptions for Active Duty family members. Thus you will still need to pick up your medications and refills at your current designated location(s).
