The Pediatric Medical Home
The Pediatric Medical Home provides outpatient medical care to eligible dependent children of active duty and retired military service members from birth to 18 years of age. We provide comprehensive, communicative, and coordinated care between the patient and the Primary Care Manager (PCM). The Medical Home is a new approach to health care delivery based on the concept that the patient is always at the center of every decision made regarding their medical care.
Pediatrics has two medical homes, the Outback Medical Home and the Safari Medical Home.
To get the most out of your medical home YOU must be ACTIVE in you or your child's care. YOU are the most important member of your healthcare team.
Before the visit:
Create a list of medications to include doses and frequency along with any questions you may have to the visit.
During the visit:
Provide or verify your medical history. ASK questions and share in the decision making process.
After the visit:
Follow your care plan carefully and keep your health care team informed.
Pediatric Medical Home Hours are from 7:30 a.m.-4:30 p.m. Monday - Friday (except holidays) with a lunch hour of Noon-1:00 p.m. . We are located in Winn Army Community Hospital next to the outpatient entrance.
